You will want to acquaint yourself with our policies, and you will want to have all the paperwork
signed and inplace before the appointment.
Reading and signing these forms before your first appointment may save you up to twenty
minutes of your appointment time. This will allow us to spend more time getting to know each
other in person.
The Patient Agreement form is our basic office policy and HIPPA acknowledgement form.
The Intake form asks for pertinent information about you and your insurance.
The History form asks basic history questions about who you are.
